Our forms are provided free of charge at our branches and by Zantray agents. You can also ask for them to be sent by email: you will receive a reply within one working day.

Account opening

Fill these in before you visit a branch to save time at the counter.

Loans

A complete application cuts the review time to 72 hours for micro-loans and school loans.

Insurance

Enrolment, claims and beneficiary designation for all our insurance cover.

Zantray Transfert

To become an agent or to dispute a transfer transaction.

How to submit an application

  1. Print or pick up the form

    At a branch, from a Zantray agent, or by email on request.

  2. Fill it in in ink, with no corrections

    A correction that has not been initialled invalidates the document. Creole and French are both accepted.

  3. Attach the required documents

    The list is on the last page of each form.

  4. Submit it at a branch

    You will be given a numbered acknowledgement of receipt: keep it, as it serves as your tracking reference.
